If you suffered a traffic accident and you have an MRI, Tomography, Ultrasound or X-ray pending through SOAT, you can contact us and schedule your appointment in an agile and timely manner.
To request an appointment you can send the following documents to the email citassoat@escanografia.com or go to the institution to schedule the appointment in person.
DOCUMENTS RELATED TO THE ACCIDENT:
- Single claim form (FURIPS) of the entity requesting the study.
- Clear photocopy of the mandatory SOAT insurance policy.
- Clear photocopy of the identity document of the victim.
- Initial emergency care and medical history related to the accident.
- Copy of the invoices or list of expenses of the IPS that have assisted you.
- Support of exams carried out in other IPS (reading).
- Photocopy of the registration.
- Accident report or complaint if the initial emergency care is not specific to validate the accident.
